SUMMARY:

Introduction: Accidents caused by snakes, especially in tropical and subtropical countries, are still a serious public health problem. Annually, there are more than 2.5 million cases of snakebite, of which about 100,000 are fatal. Local inflammation is a clearly known symptom of snake envenomation. The local and systemic inflammatory effect of snake venoms has been associated with a marked increase in the cytokines IL-1beta, IL-6, IL-8, IL-10 and TNF-alpha. Accumulating evidence indicates that these cytokines play important roles in mediating the cellular recruitment and activation necessary for inflammation and tissue repair. Considering that the hemostatic and inflammatory status of the patient can influence the making of important clinical decisions and that studies that relate these two states are still unprecedented in our country, the finding of biomarkers with prognostic value is highly desirable. Objective: To measure the plasma levels of inflammatory markers in patients who were victims of snakebites upon admission to the Emergency Care Unit. Methodology: Cross-sectional study carried out with patients who suffered snakebite treated at HPS João XXIII, in Belo Horizonte/MG. The study population consists of 11 patients victims of accidents by snakes of the genera Bothrops and Crotalus. Clinical data, at patient admission and during the monitoring period, were obtained by the Hospital team, as well as data from routine laboratory tests. The determination of plasma levels of cytokines IL-1β, IL-4, IL-6, IL-8, IL-10, IL17, TNF-α, IFN-γ, was performed by the Luminex assay. Results: The comparison between routine laboratory data of patients, before and after serum therapy, showed an increase in the number of monocytes and a reduction in MRL values; tendency towards normalization of PT/INR, aPTT and fibrinogen; in addition to a tendency to reduce IL-8, IL-1, IL-6, TNFα, IFNy, IL-17, IL-4 and IL-10 after serum therapy. Several direct and indirect correlations were also observed, that is, between some blood count data and hemostatic and inflammatory variables. Conclusion: An analysis of the data allowed the observation of several laboratory alterations after snakebite. Among these, alterations in the figurative elements of the blood and in hemostatic and inflammatory parameters, which tended to normalize after the administration of antivenom, highlighting the benefits of antivenom therapy.
