EVALUATION OF HEMATOLOGICAL PARAMETERS IN PREGNANT WOMEN WITH AND WITHOUT PERIODONTAL DISEASE

SUMMARY:

Introduction: prenatal care is of fundamental importance and, therefore, must be carried out by a multidisciplinary health team, since numerous physiological, physical, psychological and even pathological changes can be perceived in this period. In addition to medical follow-up, imaging and laboratory tests, it is important to have specific dental care, as several complications that can occur during pregnancy can have repercussions on the oral cavity. Therefore, the present study aims to analyze the hematological profile of pregnant women with and without periodontal disease (PD). Methods: This is a cross-sectional study carried out in the city of Divinópolis/MG. Data on pregnant women were obtained by consulting medical and dental records. These data were stratified into two subgroups: Pregnant women with PD (n=230) in the second and third gestational trimesters and pregnant women without PD (n=30) in the second and third gestational trimesters. Study variables were related to periodontal disease, sociodemographic, clinical and laboratory characteristics. Indices derived from hemogram parameters were calculated: LNR = Neutrophils/Lymphocytes Ratio; PLR = Platelet/Lymphocyte Ratio; MRL = Lymphocyte/Monocyte Ratio; dNLR (derived NLR) = Neutrophils / Global Leukocytes – Neutrophils; SIRI (systemic inflammation response index) = Neutrophils x Monocytes / Lymphocytes; AISI (aggregate index of systemic inflammation or aggregate index of systemic inflammation) = Neutrophils x Monocytes x Platelets / Lymphocytes; SII (systemic immune-inflammation index or index of systemic immune-inflammation) = Platelets x Neutrophils / Lymphocytes. Data were analyzed using frequency distribution, mean and standard deviation for parametric data, median and interquartile range for non-parametric data. To compare the groups, Student's t test was performed for parametric data, and the Mann Whitney test for non-parametric data. The correlation coefficient was calculated using the Pearson and Spearman correlation test. Bivariate analyzes were also performed on sociodemographic data. The analyzes were performed using the Statistical Package for the Social Sciences® software (version 19.0), with a significance level of p<0.05. Partial results: The mean age in both groups was 27 years. Regarding education and usual risk prenatal care, there was a difference between the groups with and without periodontal disease. When comparing pregnant women with PD and without PD in relation to hematological variables, only the mean corpuscular volume of red blood cells (MCV) showed a statistically significant difference between the groups. When analyzing the hematological variables of the pregnant women according to the degree of gingival inflammation, the indices of mean corpuscular hemoglobin concentration (MCHC), global leukometry and lymphocytes showed a statistically significant difference between the groups of pregnant women with gingivitis and periodontitis. Discussion: the increase in MCV can be explained by the hypersegmentation of neutrophils, which is frequent during pregnancy and may indicate a lack of folate. Other factors can also justify the lack of folate, such as an inadequate diet, physiological hemodilution, some hormones and medications. Our findings suggest that periodontitis may cause a slight increase in the number of leukocytes, a reflection of an inflammatory process affecting the periodontium. The SII showed a tendency to increase in pregnant women with PD (p=0.092), which suggests that this index may be more sensitive in detecting a chronic inflammatory state with systemic repercussions. Final considerations: the importance of the dental professional in the follow-up of pregnant women is highlighted, since a clinical evaluation is necessary to diagnose periodontal diseases.
