SUMMARY:

Diabetes Mellitus (DM) and Systemic Arterial Hypertension (SAH) when not adequately
controlled are the main underlying pathologies that lead to chronic kidney disease (CKD)
associated with risk factors such as aging, obesity, use of nephrotoxic substances and lifestyle.
As it is a disease with a long and insidious evolution, it is very important to carry out an early
diagnosis of kidney damage and adequate follow-up at all levels of health care. Clinical practice
demonstrates, however, that although all protocols are defined, in addition to clear and objective
forms of diagnosis, referral and appropriate follow-up approaches to allow access of people
with CKD or at risk of developing it to health services, health care, these people are not picked
up early by professionals from the Health Care Network (HCN) and only come for care when
they are already in the terminal phase of kidney disease and very little can be done to preserve
kidney functions. In addition to the limited literature for a better understanding of this whole
context and aware of the importance of the role of HCN in the process of health promotion,
protection and recovery, mainly to identify kidney damage early and propose approaches that
improve people's quality of life, there is a need to understand how people with CKD access the
HCN. Thus, the objective of this study is to evaluate the access of people with chronic kidney
disease to the health care network. This is a cross-sectional study developed from 2019 to 2022
in the HCN in the health services of the western health macro-region of Minas Gerais. Adults
and elderly people with CKD registered in the Unified Health System (UHS) and who were
assisted by the HCN until the indication for the replacement kidney therapy (RKT) were
included. The instrument used to investigate access was prepared from the Clinical Guidelines
for the Care of patients with CKD in the UHS and adjusted to models in the literature. It is
composed of three constructs that classify access as adequate and inadequate, they are: 1) link
with a basic health unit (UBS) 2) Access to specialized care; and 3) specialized care. Access
will be classified as adequate when, from the evaluation of the results of the three constructs,
the cumulative score reaches a value > 6 and inadequate when this value is ≤ 6. As partial results
for the qualification, a systematic review article on the topic will be presented published and
the statistical analyzes carried out to date.
