SUMMARY:

Introduction: Chronic Kidney Disease (CKD) can be defined as abnormalities in the structure and/or function of the kidneys present for three or more months, with implications for health. increased prevalence of associated diseases, such as diabetes mellitus (DM), systemic arterial hypertension (SAH) and obesity. Delay in referral to nephrology services and urgent initiation of dialysis, frequent in national clinical practice, are independent risk factors for morbidity and mortality among patients with kidney disease. The implementation of new criteria based on tools such as the Kidney Failure Risk Equations (KFRE) and multidisciplinary clinical management can mitigate inappropriate referrals and better adjust the focus of clinical care. Methods: this work was divided into two substudies with different objectives. Substudy one refers to a retrospective cohort of five years of follow-up developed in a nephrology outpatient clinic of a large hospital in a city in the center-west of Minas Gerais/Brazil, in the year 2021, whose objective was to evaluate the behavior of eGFR of chronic renal patients on conservative treatment during five years of follow-up. Substudy two is a retrospective cohort with patients from a nephrology outpatient clinic in stages 3 to 5 of non-dialytic CKD followed up for a period of at least two years to at least five years between 2014 and 2022 and who had the eGFR available in their medical records. result of the urinary albumin-creatinine ratio or Albuminuria during the follow-up period with the aim of validating the KFRE tool as a predictor of progression to renal failure. Both studies evaluated the baseline (date of insertion of the patient in the outpatient clinic and a second time point 1). The KRFE scale scores were presented using the median and interquartile range, in addition to being compared using the Mann-Whitney test. Results: substudy 1 will be presented in the format of an article already accepted and published in a qualis A4 journal entitled: Profile of estimated glomerular filtration rate in non-dialysis patients: challenge for the preventive approach. Substudy two will be presented in the form of an article to be submitted in the journal Revista de Ciências Médicas e Biológicas da UFBA Qualis A4, entitled “KIDNEY FAILURE RISK EQUATIONS tool predictive for progression of kidney disease: applicable to the Brazilian population?”Conclusions: In Brazil, public health policies, although not implemented in care practice, envisage the active search and early diagnosis of CKD in order to implement protective measures for kidney functions by a multidisciplinary health team from primary health care. For clinical practice, it is suggested that the proposed therapeutic approach is not based solely on GFR. The individualization of treatment is fundamental, considering the good practices of clinical and laboratory analysis, as well as the rapid identification of these patients for referral to primary care. In addition, the data in the medical records must be specified so that new studies with practical contributions are established for the Brazilian population. It is also worth mentioning that these authors believe that the development of studies that validate tools for individual monitoring of the risks of CKD evolution is extremely important.
