SUMMARY:

Open Dialogue (OD) is a model of immediate care for psychotic crises and other
psychological crises, in which care is offered within the first 24 hours of the first contact with
the crisis service. In this approach, the user, family and user's social network are invited to
participate in the entire treatment process. It covers a set of practices that are different from
conventional ones. We analyzed and described the activities of CAPS I in the municipality of
Carmo do Cajuru/MG on innovative practices in mental health, focusing on the
implementation of the OD based approach. It is justified by the scarcity of studies on the
approach in the country and the need for studies that address demedicalizing approaches. We
use qualitative case study methodology. Document analysis was carried out using secondary
data from the medical records of those users monitored with OD, both in the first
psychological crisis and in exacerbation, in the period from 01/01/2021 to 31/12/2021.
Interviews were carried out with professionals who worked according to the approach in the
same period, totaling 12 interviews. Analyzed 20 medical records of users in their first crisis
and 17 in re-examinations. The analysis of the interviews was carried out using the Ideas
Association Map and data triangulation was carried out. Of the total of 37 users monitored
with the AD, 54.1% were experiencing their first psychological crisis and 45.9% were in the
process of exacerbation. Of the first crises, Self-Extermination Attempts represented 25% of
cases and 35.3% of exacerbations. Psychotic crises were the cause of 10.8% of first crises and
11.8% of exacerbations. Eighty percent of the first crises stabilized within three months and
41.1% stabilized within the same period of time. Regarding medication use, psychotropic
drugs were used in 27.8% of those who were experiencing their first crisis, and were
deprescribed in 80% of these cases. From the analysis of the interviews, 04 categories
emerged: The beginning; Continuity; OD developments; Voice Listeners and Peer Support
Group, with their respective themes. It is considered that innovative practices in mental
health, focusing on OD, were implemented with challenges and difficulties inherent to mental
health care and the changes brought about in assistance marked by traditional psychiatry. We
noticed a revived hope in the reports and, mainly, in the expectation of seeing mental health
where Nothing of us, without us is really a possible reality and not a mere utopia of theorists
disconnected from daily practice.
