SUMMARY:

Anemia is characterized by the reduction of hemoglobin concentration below the expected values for age, sex, physiological state and altitude. Although it has a multifactorial cause, iron deficiency has been the main cause of childhood anemia worldwide. The global prevalence of anemia in children aged 6 to 59 months is 42.6%. In Brazil, a recent metaanalysis study estimates that the prevalence of anemia in children under seven years of age in Brazil is 33%. It is noteworthy that during the first two years of life the child grows rapidly, which requires a high supply of iron, therefore, in this age group, the child is particularly vulnerable to anemia. Although breast milk is considered the best food for the infant, recent studies indicate that exclusive breastfeeding maintained until six months or more may increase the risk of iron deficiency anemia. On the other hand, other studies show contrary results, showing that exclusive breastfeeding for longer is associated with a better iron status in children up to 24 months of age. It is understood that the effect of the duration of exclusive breastfeeding on hemoglobin levels in children at 12 months of age is not clear from the studies. The general objective of this study is to analyze the effect of the duration of exclusive breastfeeding on hemoglobin and ferritin levels in children at 12 months of age. This is a longitudinal, prospective study. Children will be followed at baseline at birth, time two to two months, time three to four months, time four to six months, time five to nine months, and time six to 12 months. The population considered for this study will consist of children born in a Hospital Center of regional health reference located in the city of Divinópolis - MG. For the sample calculation, a test power of 80%, a confidence interval of 95% and an effect size based on the prevalence of anemia in children of 20.9% were considered (BRASIL, 2009). The groups are formed by children who have been exclusively breastfed for at least 6 months (unexposed) and the other by children who have been exclusively breastfed for less than 6 months (exposed). The analysis period of 1 year is also taken into account, with 6 evaluations in this interval. Based on these parameters, the calculated sample is 327 individuals in total. Considering that this is the sample that needs to reach the end of the study, it is suggested to start with a 20% larger sample, in this case, 393 participants. The Gpower application program was used for the calculation. Regarding the field of study, data collection will be performed at baseline in the maternity ward of the Hospital Reference Center, at two and four months it will be performed by telephone call, at six months in the Basic Health Unit of reference for the child, at nine months by phone call, and at twelve months the collection will be at the Basic Health Unit of Reference of the child and in the exam laboratory of the municipality. In face-to-face follow-ups, capillary blood will be collected for hemoglobin assessment, and at 12 months venous blood will be collected for hemoglobin, ferritin and CReactive Protein assessment. In the follow-ups by telephone, the following questionnaires will be applied: Classification of the Brazilian Association of Companies and Research, Food Consumption Markers, questionnaire “Survey of wellbeing of young children”, identification form for collecting information on maternal anemia, and questionnaire on use and adhesion of prophylactic ferrous sulfate. For the analysis, the Poisson Regression Model will be used. Accompanied children will be divided into groups according to the duration of exclusive breastfeeding: - under two months/ - from two to under four months/ - four to under six months and/ - up to six months of age. The relative risk of anemia will be calculated between the different groups. The Open Source Epidemiologic Statistics for Public Health (OpenEpi) Version 3.01 and STATA Version 15 statistical software will be used to manipulate the data.
